Wrinkles on the Face: Types, Areas and Treatments
You raise your eyebrows and a line deepens on your forehead. Another remains visible even when your face relaxes. These wrinkles on the face do not all have the same origin. A wrinkle is a fold in the skin, sometimes fine, sometimes deeper. Look at whether it appears only when you move your face or whether it stays at rest: this difference helps you understand what a treatment can change.

At a glance
Five things to know before deciding- What a wrinkle is: a fold in the skin, from a fine line to a deep furrow, visible only when the face moves or also at rest.
- What causes them: repeated muscle movement, age, genetics, menopause, the sun and tobacco; natural aging cannot be changed.
- What prevention changes: it limits what speeds up skin aging, mainly the sun and tobacco; it does not erase wrinkles that are already there.
- What treatments change: relaxing a muscle, adding volume and working on the skin's surface meet different needs; no treatment is appropriate for all wrinkles, and none comes with a promise that they will disappear.
- Before any treatment: an assessment, which can lead to postponing or changing a treatment, deciding not to perform it, or doing nothing.
Recognizing the Fold and What Comes With It
A fold that appears only while a muscle contracts is a dynamic wrinkle, or expression line: it forms when you frown, smile or raise your eyebrows. A static wrinkle stays visible even without muscle activity. Over time, repeated movements and the changes that muscles and skin go through with age can cause deeper folds to develop, which no longer disappear when the face relaxes.
Fine lines are very thin lines on the surface. They are related to changes in the dermis and a drop in the water held by the epidermis, the surface layer of the skin. This is why a moisturizer can make their appearance less noticeable, without being able to fill a deep fold.
Sagging is something else: the skin has less support, the fat under it decreases and gravity lets the tissues sag. What you see then is not a fold caused by muscle movement, and this is what sets it apart from an expression line. These changes can occur together on the same face, for example an expression line that has become permanent, fine lines on the surface and the beginning of sagging.
- Dynamic wrinkle
-
Visible only when a muscle contracts.
- Static wrinkle
-
Visible at rest as well.
- Fine lines
-
Thin lines on the surface, related to the dermis and to the water the skin holds.
- Sagging
-
Loss of support and volume under the skin, along with gravity.
Why Wrinkles Appear
Collagen and elastin are two fibres of the dermis, the layer under the surface of the skin. They give it strength and elasticity. With age, these fibres decrease: the skin is less resilient, does not return to its shape as readily and loses firmness.
The pace is not the same for everyone. Genetics influences when skin changes appear. At menopause, the drop in hormones can come with skin that is drier, thinner and less firm. The sun plays a major role in premature skin aging, and tobacco speeds it up and promotes wrinkles. Finally, facial movements deepen folds where the muscles work the most.
You cannot change the natural aging process; you can, however, reduce sun exposure and tobacco use, the factors you have some control over.
- What cannot be changed
-
Natural aging and genetics.
- What can be changed
-
Sun exposure and tobacco use.
- Other factors in your life
-
Menopause and repeated facial movements.
What Prevention Changes
Prevention targets premature skin aging, not age. To protect yourself from the sun, shade, protective clothing and a broad-spectrum, water-resistant sunscreen with an SPF of 30 or higher make up everyday protection. Quitting smoking also helps, since tobacco speeds up skin aging.
A moisturizer keeps water in the skin and can make fine lines less noticeable; it does not fill a deep fold. Anti-aging cosmetics give modest results: a cream does not give you what a facelift does, and no product treats all signs of skin aging. Using many anti-aging products can irritate the skin: using more does not mean taking better care of it. Neither a food, nor an exercise, nor a home remedy is guaranteed to prevent or erase wrinkles.
Forehead Wrinkles
When you raise your eyebrows, the frontalis muscle contracts and creases the forehead crosswise. This is how horizontal forehead wrinkles form. At first, these lines appear only with movement; over time, they can stay visible at rest. Changes in the skin are then added to the movement of the muscle.
This difference matters when choosing. Relaxing the muscle reduces the contraction; adding volume lifts a fold that is already deep. These are two distinct approaches, and no forehead fold automatically calls for an injection.
An injection that relaxes a muscle carries a specific risk on the forehead: weakening the frontalis muscle can lower the eyebrow. The arrangement of the forehead muscles varies from person to person. The anatomy of your forehead and the position of your eyebrows therefore matter in choosing a treatment. Ask how this risk is taken into account before deciding.
Neck and Upper Chest Wrinkles
On the neck, two things are often confused. The inner edges of a muscle, the platysma, form vertical bands: these neck bands are related to the muscle and not to the skin, and they are not skin wrinkles. Skin wrinkles, on the other hand, are fine lines that crease the surface of the neck and upper chest. Sagging is different again: it comes from decreasing support and gravity.
Sun protection applies to the neck and upper chest too: shade, clothing that covers the skin and sunscreen add to the protection of the face. It does not make a fold that has already set in disappear.
The neck and upper chest are more likely than the face to develop lasting scarring after fractional CO2 laser resurfacing; this area calls for caution. To choose a treatment for the neck or upper chest, the doctor first tells apart a muscle band, a skin fold and sagging. For both laser treatment and intense pulsed light, whether it is appropriate for the neck or upper chest must be confirmed before considering the treatment.
Between the Eyebrows, at the Corner of the Eye, Around the Mouth
Wrinkles develop differently in each of these three areas:
Two other folds are easily confused with them. The nasolabial folds run from the nose to the corners of the mouth; a loss of volume and support can make them more prominent. Before choosing a treatment, the depth of the fold, volume and support should be considered separately: relaxing a muscle and filling a fold do not meet the same need. The corners of the mouth that droop with age, from which marionette lines start, are partly related to the muscle that pulls the corner of the mouth down and to the skin's loss of elasticity: this is not the same thing as the fine wrinkles that crease the lips.
- Between the eyebrows
-
The frown lines, vertical folds that frowning creates and that make you look angry or worried when you are not.
- At the corner of the eye
-
The crow's feet, fan-shaped lines seen mostly when you smile.
- Around the mouth
-
The wrinkles around the mouth, which appear with age when the muscle surrounding the mouth contracts.
Which Treatment Is Appropriate for Each Need?
No treatment is the most effective for all wrinkles: no product treats all signs of skin aging, and each treatment meets a specific need. The most useful approach is to start from what you see.
A fold that movement deepens. Among cosmetic injections, the one that relaxes a muscle aims to reduce wrinkles by blocking the contraction of the facial muscles under the skin. It therefore acts on movement, and it is less effective on a static wrinkle than on a dynamic one. A physician, or a nurse injector supervised by a physician, performs it.
A line or furrow that lacks volume. The injections that add volume can lift a line or furrow. Their action differs from that of an injection that relaxes a muscle: one adds volume, the other reduces movement, and one does not replace the other for the same fold.
Laser treatment: its suitability must be confirmed. The fractional CO2 laser creates small breaks in the skin barrier, with a risk of infection. CARE MD also offers ablative CO2 laser resurfacing; this does not mean it is appropriate for your wrinkles. Whether it is appropriate must be confirmed before choosing it for this need. A physician, or a nurse supervised by a physician, performs these laser procedures. The type of fold, the condition of your skin, how it heals and the recovery time you accept are points to discuss before any decision.
Ultrasound: acting below the surface. Microfocused ultrasound produces small zones of heat injury at depths set in advance: it is not a surface treatment. The results and how long they last depend on the treatment being considered; during the assessment, the doctor clarifies what is reasonable to expect and the after-effects to plan for. The areas for which the device used is authorized and its limitations must be checked before choosing a treatment for the face or neck.
Intense pulsed light. CARE MD also offers this treatment. Before choosing it, ask which skin problem it targets and whether its use is appropriate for this area and your expectations.
- Fold that movement deepens
-
Relaxes the muscle; less effective once the fold has set in at rest.
- Line or furrow that lacks volume
-
Adds volume; does not act on the movement of the muscle.
- CO2 laser
-
It breaks the skin barrier; whether it is appropriate for wrinkles must be confirmed, and healing matters in the decision.
- Microfocused ultrasound
-
Heat acting below the surface, distinct from a surface treatment; the authorized areas and the limitations of the device should be checked.
- Sagging
-
It is related to the support of the skin and to gravity; a cream does not give you what a facelift does.
Wrinkle treatment is only one part of facial rejuvenation, which also includes skin texture and quality.
Without Injections, Without Surgery, Quickly: What to Expect
Without injections. Prevention, hydration and cosmetic skin care remain possible, and a surface treatment can be discussed during the assessment. None of these treatments relaxes a muscle: they do not act on the movement that deepens the fold, and anti-aging cosmetics give modest results. To decide without injections, clarify what you want to change: the appearance of a fine line, a deep fold or the movement that deepens it. Hydrating the skin does not fill a furrow and does not block the contraction of a muscle.
Without surgery. A non-surgical treatment is neither free of after-effects nor free of risk. For example, fractional CO2 laser breaks the skin barrier, with a risk of infection. A cream does not give the results of a facelift, and choosing no surgery does not guarantee the same change. If surgery of the face and neck is appropriate for your situation, the doctor will guide you toward it.
Quickly. No one can promise that a wrinkle will disappear quickly. Four aspects should be considered separately: the time before the effect appears, how long it lasts, recovery after the treatment, and the number of sessions or the maintenance. They depend on the treatment and the person, and the doctor clarifies them during the assessment for the treatment being considered.
Why an Assessment Comes Before Treatment
The same fold can have different causes and call for different treatments. To prepare for the assessment, note whether the fold stays at rest or appears with movement, and what you want to change. Bring your questions about your skin and the position of your eyebrows, along with information about your medical history, medications and supplements, previous treatments and allergies. Before a cosmetic injection, you need to know the risks and benefits, and consult an experienced professional.
During the assessment, the doctor may conclude that it is better to postpone a treatment, choose another or do nothing. An injection that relaxes a muscle is not performed on skin that is inflamed or infected, or if there is hypersensitivity to an ingredient. Also mention whether you are pregnant or breastfeeding. Every medical procedure carries risks, and the level of risk you accept is up to you: nothing obliges you to treat a wrinkle. Ask which observations require an in-person examination before choosing the treatment.
Risks and After-Effects by Treatment
- Injection that relaxes a muscle
-
On the forehead, the eyebrow can drop; difficulty swallowing, speaking or breathing requires immediate medical attention.
- Injection that adds volume
-
It lifts a line or furrow instead of relaxing a muscle. Before deciding, ask about the temporary effects, the serious complications and the signs that require immediate help for the filler being considered.
- Fractional CO2 laser
-
The skin barrier is broken, with a risk of infection; on the neck and upper chest, scarring is more of a concern than on the face.
- Ultrasound
-
The after-effects to plan for depend on the treatment being considered; ask which effects to expect and what recovery to plan for with the device used.
- Anti-aging cosmetics
-
Several products at once can irritate the skin.
Seek care right away
Difficulty swallowing, speaking or breathing after an injection that relaxes a muscle requires immediate medical attention: do not wait to contact CARE MD first. Before any other treatment, ask for specific instructions on recognizing its complications and knowing where to get help, including outside opening hours.

Frequently Asked Questions
Is there a miracle cure for wrinkles?
No. You cannot change the natural aging process, and no product treats all signs of skin aging. A fold that movement deepens, a fold that has set in at rest and fine lines do not call for the same treatment. Sagging can come with them, but it is not a wrinkle. A cream does not give you what a facelift does, a non-surgical treatment has its own after-effects, and no one can promise that a fold will be erased or will disappear quickly. What helps is more modest: protecting yourself from the sun, quitting smoking, hydrating the skin and, if a fold bothers you, discussing it during an assessment to see what is appropriate, or whether it is better to do nothing. Neither a natural remedy, nor a food, nor an exercise is guaranteed to erase wrinkles.

What is the most effective treatment for wrinkles?
There is no single one, because wrinkles do not all have the same cause. An injection that relaxes a muscle targets folds that movement deepens and is less effective on a wrinkle that has set in at rest. An injection that adds volume lifts a line or furrow. These two approaches have different limitations. CO2 laser cannot be chosen as the answer to your wrinkles before it is confirmed that it is appropriate; ultrasound acts below the surface and is not a surface treatment. The best choice depends on what you see, on your skin and on the after-effects you accept; it is made during an assessment, and doing nothing is one of the options.

Can you reduce wrinkles without surgery?
Yes, some options do not require surgery. Hydration can make fine lines less noticeable; an injection that relaxes a muscle acts on movement, while filler lifts a line or furrow. These approaches do not meet the same need. Non-surgical does not mean without after-effects or without risk, and a cream does not give the results of a facelift. Whether CO2 laser is appropriate for wrinkles must be confirmed before choosing it; the fact that a device is offered is not enough to make it the right choice for you. If surgery of the face and neck is appropriate for your situation, the doctor will guide you toward it.

Can you reduce facial wrinkles quickly?
No one can promise that a wrinkle will disappear quickly. They vary from one treatment to another and from one person to another, and the doctor clarifies them during the assessment for the treatment being considered. Prevention, on the other hand, starts right away: sun protection and quitting smoking limit what speeds up skin aging.

What are the different types of wrinkles?
A dynamic wrinkle, or expression line, appears while a muscle contracts. A static wrinkle stays visible at rest. Fine lines are finer, related to changes in the dermis and a drop in the water the skin holds. Sagging is different: the tissues have less support and volume, and gravity lets them sag. It can occur together with folds on the same face. Over time and with age, an expression line can become visible at rest.

Can you treat wrinkles without injections?
In part. Prevention, hydration and cosmetic skin care remain possible, and a surface treatment can be discussed during the assessment. None of these treatments relaxes a muscle: they do not act on the movement that deepens a fold, and anti-aging cosmetics give modest results. The assessment is an opportunity to see what remains useful for your skin, what you expect and what each option does not change.

What is the definition of a wrinkle?
A wrinkle is a fold in the skin, ranging from a fine line to a deep furrow. Some appear only with facial expressions, while others stay visible at rest. They are related to age, genetics, the sun, tobacco and the repeated movement of the facial muscles, and they can come with sagging skin.

Wrinkles on the Face: What Treatments Do Not Guarantee and What Calls for Caution
At CARE MD, the doctor assesses what is appropriate for you: your needs are your own.
Limitations and precautions
Tell the doctor about your medical history, allergies, medications, supplements and previous treatments, and whether you are pregnant or breastfeeding.
Do not have an injection that relaxes a muscle if your skin is inflamed or infected, or if you are hypersensitive to an ingredient.
Choose a professional who is experienced in injections, and understand both the benefits and the risks before receiving the treatment.
After an injection that relaxes a muscle, get immediate medical help if you have difficulty swallowing, speaking or breathing.
To protect yourself from the sun, stay in the shade, cover your skin and apply a broad-spectrum sunscreen.
You cannot expect a guarantee that your wrinkles will disappear, or a treatment that is superior for every wrinkle.
An injection that relaxes a muscle has less effect on lines at rest than on expression lines; on the forehead, your eyebrow can drop.
With fractional CO2 laser, your skin loses its protective barrier, which creates a risk of infection; the risk of scarring is higher on the neck and upper chest than on the face.
Microfocused ultrasound heats below the surface, not on the surface: have the authorized areas and the limitations of the device checked.
With anti-aging cosmetics, expect a modest improvement.
